Social Work Services
Our team of social workers conducts regular visits with clients and consults with our meal delivery drivers, who see clients on their daily routes.
When a MOWSF client could benefit from additional support:
- we make referrals to other services,
- assist with crises,
- and provide short-term support services.
Our commitment to integrated care creates a link between the meals we provide and other services that a senior may need in order to remain independent.
SOCIAL WORK VISITS
A MOWSF social worker visits each new senior client at the beginning of service to evaluate his or her needs. They then provide home visits on a yearly basis to assess each client’s needs and satisfaction with our services.
Our professional, culturally competent social workers are well-informed on the resources available for serving homebound seniors — particularly the coordinated community-based services so many seniors rely on to help them maintain their independence.
Our social workers help clients by facilitating referrals to services including:
- home care
- benefit application assistance
- legal resources
Our social workers meet regularly with our meal delivery drivers on their mutually assigned routes to assess how the clients are doing. If a client needs support management or volunteer services, the social worker will follow up.
Our social workers are available for support management services for our clients who are most at risk of losing their ability to live on their own and who are without access to other support management services. We develop a plan with the client and give them the tools to address the immediate situation and future concerns.
ADDITIONAL CITYWIDE SERVIECS
The San Francisco Department of Aging and Adult Services (DAAS) coordinates services to older adults, adults with disabilities, and their families. Their goal is to maximize self-sufficiency, safety, health, and independence so that seniors can remain living in their communities for as long as possible and maintain the highest quality of life.
